Sea Queen, a takeaway at 7 Rookery Lane, Lincoln, Lincolnshire was given the score after assessment on June 29, the Food Standards Agency's website shows.
It means that of Lincoln's 121 takeaways with ratings, 87 (72%) have ratings of five and none have zero ratings.
