Lotus House, a takeaway at 303-305 Newark Road, Lincoln, Lincolnshire was given the score after assessment on July 7, the Food Standards Agency's website shows.
It means that of Lincoln's 121 takeaways with ratings, 87 (72%) have ratings of five and none have zero ratings.
