COLUMN: This is our new model of care

I write this month, about the Lincolnshire Health and Care Case for Change report that was shared with the public recently.

If you didn’t manage to catch sight of this, the report states that our health and care system is spending £60 million more a year than it receives in funding and income. The growing demand for health and care services has led to a situation where services are struggling to deliver consistent, high quality care. In Lincolnshire, particularly, we struggle to meet local demands and are regularly challenged by workforce shortages.

I would like to reiterate that, at LCHS, our priority will always be to provide the highest quality, safest services for our patients within the resources available to us. However, we must make changes if the service we provide is to remain sustainable.

As part of our plans we will be transforming community based services to ensure people get better, more personalised and more integrated care that supports them to stay at home for as long as possible and keep them out of hospital.

We will be focused on building seven-day-a-week services for local people through community ‘neighbourhood’ teams, which will be supported by urgent care centres. This will allow hospitals to be more available for genuine emergency and time critical services.

Our new model of care has been developed by doctors, nurses and professionals, with input from the public and patients, through engagement with more than 15,000 people over two years. We will continue to engage with you all over the coming months, and I look forward to keeping you up-to-date on our progress.
