COMMUNITY: First Contact Signposting Scheme

The scheme uses a simple checklist to help people aged 60 or over receive services to keep them safe and independent in their own homes.

If a staff member from any of the partner agencies, such as a firefighter, police officer or volunteer goes into an older person’s home, they will complete a checklist to find out if they have any other needs for:

A home security check.

Any repairs or mobility adaptations that need doing in the home.
