Notts County Council is working with the six local Clinical Commissioning Groups (CCGs) covering the county to make sure that health and social care services are efficient and joined up wherever possible to improve the way older people receive care and support. The Better Care Fund 2015/16 Plan will enable the partners to pool £59m of their money together to connect health and social care services more effectively.

Coun Joyce Bosnjak, chair of Notts County Council’s Health and Wellbeing Board, said: “We have all seen the national media headlines about hospitals struggling with an increasing demand for beds over the winter period and the County Council is working with our partners in the NHS to move towards more joined up services that will benefit the people of Notts more.”
